NARRATIVE: ON MAY 23, 1996, AT APPROXIMATELY 2500 HOURS (ZULU) A BEECHCRAFT, MODEL BE-1900-D, N148SK, FLIGHT NUMBER 1199, RETURNED TO INDIANAPOLIS INTERNATIONAL AIRPORT (IND) SHORTLY AFTER TAKEOFF REPORTING SMOKE IN THE COCKPIT/CABIN AREA. THE AIRCRAFT LANDED ON RUNWAY 05L. 18 PASSENGERS AND 2 CREW MEMBERS EVACUATED AIRCRAFT ON RUNWAY. INDIANAPOLIS TOWER CLOSED RUNWAY 05L FOR A SHORT PERIOD OF TIME. RAYTHEON CORPORATION OF INDIANAPOLIS REMOVED THE AIRCRAFT FROM THE RUNWAY AND TOWED IT TO THEIR RAMP AREA. NO INJURIES WERE REPORTED. THE AIRCRAFT IS OWNED AND OPERATED BY ASTRAL AVIATION, INC., DOING BUSINESS AS SKYWAY AIRLINES. THE COMPANY IS BASED OUT OF MILWAUKEE, WISCONSIN.

SKYWAY AIRLINES ACFT WAS AT 3,000 FEET PREPARING FOR APPROACH WHEN IT STRUCK A 20 POUND GOOSE WITH THE RIGHT WING, THE GOOSE PASSED THROUGH THE LEADING EDGE, THROUGH A FUEL CELL, THROUGH A WING SPAR, AND LODGED JUST IN FRONT OF THE AILERON. THE ACFT LANDED SAFELY WITH ONE ENGINE OUT. NO INJURIES WERE REPORTED. THE ACFT HAS SUBSTANTIAL DAMAGE. (.19) ON FEBRUARY 8, 1999, AT 1135 CENTRAL STANDARD TIME (CST), A BEECH 1900D, N148SK, PILOTED BY AN AIRLINE TRANSPORT PILOT, SUSTAINED SUBSTANTIAL DAMAGE FOLLOWING AN IN-FLIGHT IMPACT WITH A CANADIAN GOOSE. INSTRUMENT METEOROLOGICAL CONDITIONS PREVAILED AT THE TIME OF THE ACCIDENT. THE FLIGHT WAS OPERATING UNDER THE PROVISIONS OF 14 CFR PART 121 AS A PASSENGER CARRYING FLIGHT. THE FLIGHT WAS OPERATING ON AN INSTRUMENT FLIGHT PLAN. THE TWO CREW MEMBERS AND TWELVE PASSENGERS REPORTED NO INJURIES. THE FLIGHT DEPARTED KENT COUNTY INTERNATIONAL, GRAND RAPIDS, MICHIGAN, AT 1053 CST, AND WAS EN ROUTE TO GENERAL MITCHELL INTERNATIONAL, MILWAUKEE, WISCONSIN. ACCORDING TO THE CREW'S WRITTEN STATEMENT, THEY REPORTED THAT APPROXIMATELY 24 NAUTICAL MILES FROM THE FLIGHT'S DESTINATION, AT 2,900 FEET MEAN SEA LEVEL (MSL), THEY ENCOUNTERED A FLOCK OF 20 TO 30 CANADIAN GEESE WHILE THE AIRCRAFT WAS FLYING IN A BROKEN CLOUD LAYER. THE CREW STATED THAT ONE GOOSE IMPACTED THE OUTBOARD LEADING EDGE OF THE RIGHT WING, PUNCTURING A HOLE THROUGH THE LEADING EDGE AND RUPTURING HTE FUEL TANK. THE CREW STATED THAT THE AIRCRAFT BEGAN TO SHAKE AND THE CREW AGREED TO SHUT DOWN THE RIGHT ENGINE. THE CREW DECLARED AN EMERGENCY WITH AIR TRAFFIC CONTROL (ATC) AND WAS ABLE TO COMPLETE THE FLIGHT TO GENERAL MITCHELL INTERNATIONAL. (-23)ON FEBRUARY 8, 1999, AT 1130, SKYWAY AIRLINES FLIGHT 1123, A BEECH 1900D, N148SK, ENROUTE FROM GRR TO MKE WAS DESCENDING THROUGH 3000' AGL FOR AN APPROACH INTO MILWAUKEE (MKE). AT APPROXIMATELY 24NM NORTHEAST OF MKE THE AIRCRAFT ENCOUNTERED A FLOCK OF CANADIAN GEESE, ONE OF WHICH IMPACTED THE LEADING EDGE OF THE RIGHT WING APPROXIMATELY 6 FEET INBOARD OF THE WING TIP. THE AIRCRAFT SUSTAINED SUBSTANTIAL DAMAGE, WITH THE IMPACT CREATING A HOLE, THE SIZE OF THE GOOSE, THROUGH THE LEADING EDGE OF THE WING CARCASS BEING BISECTED BY A WING RIB. FURTHER PENETRATION OCCURRED THROUGH LIGHTING HOLES OF THE FRONT SPAR; A RIGHT WING FUEL CELL; THE REAR SPAR; WITH THE REM AINDER OF THE CARCASS ULTIMATELY COMING TO REST IN THE AFT AREA OF THE WING FORWARD OF THE RIGHT AILERON. AN EMERGENCY WAS DECLARED AND THE AIRCRAFT MADE AN UNEVENTFUL LANDING ON RWY 19R WITHOUT ANY FURTHER COMPLICATIONS.

Description
Shortly after taking off, the crew of the airplane noted smoke in the cabin, and they returned and landed at the departure airport. Investigation revealed that a bearing in the forward cabin ventilation blower motor had failed. There was evidence that after the bearing had failed, the wires and resistors to the ventilation motor had overheated. The wires were charred and sooted on the blower motor housing. The resistors (for low speed fan operation) showed signs of heat damage. Current limiters were provided to protect the electrical circuit. However, research revealed that the wiring and resistors were not adequately protected in the low speed fan mode.

The crew reported that approximately 24 nautical miles from the flight's destination, at 2,900 feet mean sea level, they encountered a flock of 20 to 30 Canadian geese while the aircraft was flying in a broken cloud layer. The crew stated that one goose impacted the outboard leading edge of the right wing, puncturing a hole through the leading edge and rupturing the fuel tank. The crew stated that the aircraft began to shake and the crew shut down the right engine. The crew declared an emergency with Air Traffic Control and was able to complete the flight to General Mitchell International.
