March 28, 2002 - CINCINNATI, Ohio

Description
(.19) ON MARCH 28, 2002, ABOUT 1500 EASTERN STANDARD TIME, A PIPER J4A, N27838, WAS SUBSTANTIALLY DAMAGED DURING A FORCED LANDING AFTER TAKEOFF FROM CINCINNATI MUNICIPAL-LUNKEN FIELD, CINCINNATI, OHIO. THE CERTIFICATED PRIVATE PILOT WAS NOT INJURED. VISUAL METEOROLOGICAL CONDITIONS PREVAILED AT THE TIME, AND NO FLIGHT PLAN WAS ON FILE FOR THE LOCAL FLIGHT. THE PERSONAL FLIGHT WAS CONDUCTED UNDER 14 CFR PART 91. ACCORDING TO A FEDERAL AVIATION ADMINISTRATION (FAA) INSPECTOR, THE PILOT REPORTED THAT THE ENGINE STOPPED AS THE AIRPLANE WAS CLIMBING THROUGH 500 FEET. THE PILOT THEN ATTEMPTED TO MAKE A FORCED LANDING BACK TO THE AIRPORT. DURING THE RETURN, THE LANDING GEAR STRUCK THE EDGE OF THE RUNWAY, THE LEFT GEAR COLLAPSED, AND THE AIRPLANE FLIPPED OVER. THIS WAS THE FIRST FLIGHT AFTER THE ANNUAL INSPECTION. DURING THE INSPECTION THE CARBURETOR WAS REPLACED AND THE FLOAT IN THE CARBURETOR HAD ALSO BEEN CHANGED. POST ACCIDENT EXAMINATION REVEALED THAT THE CARBURETOR FLOAT WAS OUT OF ADJUSTMENT. (-23) PILOT DEPARTED RUNWAY 21R AT LUK. AT APPROXIMATELY 400 FT. THE ENGINE LOST POWER. THE PILOT ATTEMPTED A LEFT 270 TURN. THE AIRCRAFT LOST ALTITUDE AND THE LEFT MAIN WHEEL ASSEMBLY STRUCK THE EDGE OF THE RUNWAY AND DEPARTED THE AIRCRAFT. THE AIRCRAFT CONTINUED SKIDDING ACROSS THE RUNWAY AND CAME TO REST INVERTED IN THE GRASS ADJACENT TO THE RUNWAY. THE PILOT WAS THE SOLE OCCUPANT AND WAS NOT INJURED.

The solo student pilot of the tailwheel-equipped airplane reported that, during final approach to land, he realized the airplane was too high, so he performed a forward slip. During touchdown, the airplane bounced twice and veered right. The airplane ground looped to the right, exited the runway, and the right main landing gear struck a guard rail. The airplane sustained substantial damage to the right wing lift strut. The student reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

Description
Shortly after takeoff, while the airplane was climbing through an estimated 300 feet, the engine lost power. The pilot saw he had terrain obstructions straight ahead and to the right, and jet traffic on the ground to the left. He attempted to land between parallel runways, but approaching the touchdown area, saw there was unsuitable terrain in the form of bogs and ditches. The pilot then continued to turn another 90-120 degrees to the left. The airplane "ran out of airspeed," and the left wheel caught the side of the runway. The left main landing gear collapsed, and the airplane flipped over. This was the first flight after an annual inspection. During the inspection, the carburetor was replaced, and the carburetor float had also been changed. Post-accident examination revealed that the carburetor float was out of adjustment.
